What the Registration Covers

• Patient Information: Basic contact details, emergency contact, and referral source
• Medical History: Current medications, allergies, and health conditions that may affect your dental care
• Dental History: Previous dental work, concerns, and treatment goals
• Insurance Information: Dental insurance carrier, group number, and subscriber details
• Consent and Privacy Forms: Treatment consent and HIPAA privacy acknowledgment

What to Bring to Your First Visit

• Photo ID (driver’s license or state ID)
• Dental insurance card
• List of current medications and dosages
• Any recent dental X-rays from a previous provider (we can request these on your behalf)

Insurance Verification

Transferring Your Records

Switching from another dentist? We make the transition easy. With your written authorization, we can request your dental records and X-rays from your previous provider. This helps Dr. Kim review your dental history and ensure continuity of care from day one.
